Melatonin: A Clock Signal, Not a Sleeping Pill

Melatonin is the most misunderstood substance in the sleep aisle. People take it like a sleeping pill, at sleeping-pill logic (didn’t work, double it), for a problem it was never going to fix. The trial data is actually clear once you ask it the right question.

What the meta-analysis shows

Ferracioli-Oda’s meta-analysis pooled 19 randomised placebo-controlled trials, 1,683 participants with primary sleep disorders. Result: sleep onset latency down about 7 minutes, total sleep time up about 8, and a modest but significant improvement in overall sleep quality. The effects did not fade with continued use, which distinguishes melatonin from most sedatives.

Seven minutes. If a sleeping pill delivered seven minutes, it would be pulled from the market. The authors said as much: the absolute benefit is smaller than prescription hypnotics, and the case for melatonin rests on its benign side-effect profile, not its power. Anyone selling you melatonin as a knockout is selling against the data.

Why 7 minutes is the wrong number to judge it by

Here is the reframe that makes melatonin make sense. Your pineal gland releases melatonin in the evening as a darkness signal: it tells the circadian system that night has begun. Swallowing melatonin is not sedation; it is sending that signal manually.

If your clock is already correct, sending the signal again buys you the 7 trivial minutes above. If your clock is wrong, shifted late from evening screens, a flight across time zones, a chaotic schedule, the signal is exactly what is missing, and the effect is much larger than the all-comers average suggests. This is why melatonin’s best evidence sits in circadian problems: jet lag, delayed sleep phase, and to a degree shift work.

Practical translation. Falling asleep at 2 am and wanting 11 pm: good use case. Waking at 3 am after wine: wrong tool, that is alcohol’s rebound. Poor sleep in a 24°C bedroom with a 4 pm espresso habit: fix the basics, no molecule outruns them.

Dose: why less is more

Physiological night-time melatonin levels are produced by tiny amounts; oral doses of 0.3 to 0.5 mg already push blood levels into and beyond the natural range. The 5 and 10 mg tablets dominating shop shelves are pharmacological overkill that mostly buys you higher morning levels, next-day grogginess in some people, and receptor exposure far above anything your pineal ever produced.

The dose-finding logic that follows from mechanism: start at 0.5 to 1 mg. If the problem is genuinely circadian and correctly timed melatonin does nothing at 1 mg, 10 mg is unlikely to rescue it, and the diagnosis deserves another look before the dose does. Higher doses have trial precedent and short-term safety, but “more” is not how a signalling molecule improves.

One more shelf note: in the EU, products above 0.5 mg per daily dose sit in a regulatory grey zone that varies by country, and in Poland melatonin is sold both as supplement and as registered drug. The registered-drug versions have the more reliable content, which matters more than it should, as the next section shows.

The label problem

Melatonin supplements have a documented accuracy problem: analyses of retail products found actual content ranging from a fraction of the label claim to several times it, sometimes varying between batches of the same product. When a 3 mg gummy can contain anywhere from under 1 mg to over 10, dose-finding on gummies is guesswork.

The fix is boring: buy melatonin as a registered pharmaceutical where available, or from manufacturers publishing batch certificates, and prefer plain tablets over gummies, which test worst. This is the rare supplement where the pharmacy version is both cheaper and better than the wellness version.

Timing rules

For sleep onset tonight: 0.5 to 1 mg, 30 to 60 minutes before your intended bedtime, lights already dimmed, because bright evening light suppresses the very signal you just swallowed.

For shifting a late clock earlier, the dose moves earlier too: small doses taken 2 to 4 hours before your current natural sleep time pull the rhythm forward more effectively than the same dose at bedtime. Combine with the morning-light and fixed-wake-time steps of the sleep protocol, which push the clock from the other end; the combination is the actual treatment, melatonin is one lever of it.

For eastbound jet lag: at destination bedtime for the first few nights. Westbound usually needs no melatonin at all, just evening light.

A two-week phase-shift protocol

For the commonest real use case, a clock stuck 2 to 3 hours too late, here is the whole plan on one card. Assume you currently fall asleep at 1:30 am and want 11 pm:

  1. Fix the wake time first, seven days a week, at the target (say 7 am), tolerance twenty minutes. This is non-negotiable and does most of the work.
  2. Ten minutes of outdoor light within an hour of waking, every day.
  3. Melatonin 0.5 mg at 10 pm for the first week, which is roughly 3 hours before your current sleep onset, then hold at 10 pm as your sleep time drifts toward it.
  4. Overhead lights off and screens dimmed from 9:30 pm; evening light is the counter-signal and will cancel the tablet.
  5. Log minutes-to-sleep daily; expect the drift to show in days 4 to 10, not on night one.

After two weeks, most late-shifted sleepers no longer need the tablet; the light and wake anchors hold the new phase on their own. If nothing moved despite honest execution, the problem probably is not phase, and the screening questions apply.

Safety and who should check first

Melatonin’s headline safety is genuinely good: no dependence, no meaningful rebound insomnia in trials, mild and transient side effects (grogginess, vivid dreams, headache) as the common complaints. That is the strongest argument it has, and it is a real one.

What we do not know

Long-term daily use in healthy adults has surprisingly thin data past a few months; nothing alarming has surfaced, but “no signal” and “well studied” are different statements. Whether chronic supraphysiological dosing changes receptor sensitivity in humans is unresolved. And the popular claims beyond sleep, antioxidant, longevity, immune, run far ahead of human trial support; on this site those get the label they deserve: mechanistic.

Where to start

Ask what problem you are solving. If it is the clock, 0.5 to 1 mg of pharmacy melatonin, correctly timed, alongside morning light, for two weeks with a written log. If it is not the clock, magnesium repletion or glycine are more honest bets, and the basics beat all three.

Is it safe to take melatonin every night?

Trial data up to a few months shows no dependence and no rebound insomnia, with mild side effects. Long-term daily data beyond that is thin rather than worrying. Keep the dose low, check interactions if you take anticoagulants, immunosuppressants, epilepsy or diabetes medication, and treat use in children as a doctor's call.

Why does melatonin not work for me?

Three usual reasons: the problem is not circadian (a hot bedroom, late caffeine or evening alcohol will beat any dose); the timing is wrong (for shifting a late clock, take it 2 to 4 hours before your current sleep time, not at bedtime); or the product does not contain the labelled dose, which analyses show is common, especially in gummies.
